The Role of T2-Gradient Echo MRI in a Case of Infective Endocarditis

Abstract
T2*-gradient echo (GRE) MRI frequently detects multiple asymptomatic cerebral microbleeds in infective endocarditis (IE) patients that have developed cerebral infarction or other neurologic complications. A 63-year-old was admitted to hospital be-cause of left hemiplegia and dysarthria accompanied by fever. MRI showed multiple cerebral infarctions. GRE MRI showed mul-tiple microbleeds in areas of cerebral infraction, adjacent to ischemic lesions, and higher cortical lesions. Blood culture revealed group-D streptococcus, and he was diagnosed as having multiple cerebral infarctions due to IE. After antibiotics therapy, he under-went open heart surgery for severe mitral regurgitation without sequelae. We suggest that GRE MRI provides a noninvasive means of diagnosing cerebral infarction associated with IE. (Korean J Stroke 2008;10:62-64)
